Many people are surprised to learn this: shoulder pain is rarely only a “shoulder muscle tear” or “tendon injury.” Even when scans show a tear, many similar-looking injuries exist in people who have no pain at all. That’s because pain, stiffness, weakness, or instability often come from the system around the shoulder, not only from one structure. Nerves like the axillary nerve, suprascapular nerve, and neck nerve roots from C5–T1 strongly influence shoulder sensation, muscle strength, and movement confidence.
